Applications will open in June 2026

In order for each child to be considered for admissions into E5 Social, they must have 1-3 sponsors fill out the following form. Sponsors are any non-related adult who has know the child for at least 3 months, and believes the child will greatly benefit from E5 Social. Please fill out the from in its entirety, in as much detail as possible.

Note: Our goal is to really get to know the child. We need as much detail as possible. We are not looking for perfect teens, far from it actually. We need to be as prepared as we can be in order to meet the needs of each child. Thank you for your assistance and for being a presence in a child’s life.
